  • Correspond to International Student Adviser ( ISA)
  • Get an offer letter from your Employee/Consultancy
  • Fill out 1-765 Form. ( here you need to provide E verification Code of Employee/Consultancy)
  • Submit it to International Student Office ( ISO)
  • ISA will issue new I20
  • Get the new I20 .
  • Submit I765 Form and Check
or money order in the amount of $340.00, payable to U. S. Department of
    Homeland Security.  See Form I-765
Instructions page 8 for more information.

These above steps will provide you basic ideas about your OPT extension. The following checklist  can be also helpful to you. My advise for you is to communicate with your ISA/ISO.

17-Month STEM Extension

Optional Practical Training (OPT) Tutorial

Application Materials

 Unlike
applying for the initial 12 months of OPT, you
are responsible for assembling the entire OPT extension application.
The following documents are required as
stated in the Form I-765 Instructions on the www.uscis.gov
website, pages 3 and 6:

 1.    Check or money order in the amount
of $340.00, payable to U. S. Department of Homeland Security.  See Form I-765 Instructions page 8 for more
information.

2.
Completed Form I-765, available on
the USCIS website, www.uscis.gov.

i.
Follow all directions when typing
form.  Family name should be listed first
in CAPITAL letters, followed by your first name.

ii.
Question
#16 will be the code (c) (3) (C), exactly as shown.

iii.
Question #17 must be completed
including employer’s E-Verify number.

iv.
Print the form and sign your name in
blue ink.

      3.     Copy of
your degree.
SEVP and USCIS have stated that transcripts or copies of
diplomas that indicate the level of the degree and the field of study would be
acceptable as proof of your degree.

4.     An endorsed I-20 from the
Office of International Education recommending the OPT extension.

5.     Copy of your current OPT EAD
card (front and back).

6.     Two identical photos taken
within the last 30 days on a white background.
Please see the following
internet address for picture specifications: www.travel.state.gov/passport/guide/composition/composition_874.html.
Stores such as Fedex-Kinkos and
Walgreens generally offer passport photo services.

7.     Copy of most recent Form
I-94 (front and back).

8.     Copy of passport
picture page/expiration date page (must include name).  Passport must be valid.

9.     Copy of current
visa page.  It is okay if your F-1 visa
has expired.

Mailing
the Application:
Once you have all the required
documents, you will mail them according to the Form I-765 Instructions on
page 10 which refer to the chart on page 11.
The state you currently live in determines
where the application will be mailed.

PLEASE
NOTE: We recommend mailing the
application by CERTIFIED MAIL, RETURN-RECEIPT REQUESTED.  This way, the USCIS Service Center will have
to sign a receipt card that will then be mailed back to you as proof that they
received your application.

Hi marulez,
I think you have applied for the extension. I am on extension and I wanted to ask you about your job. Are you employed or did extension through a consultancy.
I have also started a new thread on " Payroll during OPT extension".
If you can go through that and give some suggestions. If you are also in same boat as mine, that might be helpful for you too.'
